Before we started these lessons I always thought that knowledge and information were completely different and could be divorced. However, this week I have learned that what data means. Davenport and Prusac, 1998, pg 2 describes that data is a set of discrete, objective facts about events.
What I understood from this lecture is that the TPS, DSS, MIS and ELS work with the communication and collaboration as well as an office system, which are used in the framework for information system.
Now, I understand that information is data endowed (have something as a nature) with relevance and purpose. Knowledge cannot be divorced from information because if you don t have data and information then you can’t have knowledge of something.
Discuss how you would relate theory to practice
If we look at the table of the historical perspective, it makes it clear for us that the most important organisation factor is collaboration (communities) in knowledge. The technology can still be used in the same way for learning, innovating, consulting, collaborating which then helps us to get a good idea for all the obstacles we get in our way in everyday life.
The information system building blocks, we understand that how the application is used in the framework of information technology because it is an architectural look. The architectural focused on different views of information technology in the various stakeholders.[1]
For example, as we know that the building blocks were divided into four information systems which are Data, Information, knowledge, and wisdom. In my opinion Data is a fundamental goal in an Information system because if we look at any organization, especially modern organization is not interested in raw data instead of they are interested in information that bring a new business knowledge of technology and it will help managers to make a good decision as well as will the support mission statement of the organization.[3]
Another example is, in the knowledge stage, the organisation system is concerned with the database technology ,which will be used in the second stage( Information) to support the business knowledge because the system translate the data requirements into database information, which is used by the knowledge system builder. Then a system builder used the knowledge to manage by customise the data, test, implemented and make an intelligent decision. [2]
Application: How does the theory and practice relate to the case study?
The Fumble medical practise need to understand organisation, hospitals, national government and chariots. Those are enable manager to understand the issues, problems and learn from examining the behaviour of other organisation as well as identify possible solution based on what’s done elsewhere.
Data information and knowledge relates highly to the case study. However, this takes a lot of experience and knowledge. For example, all their names, age’s, telephone numbers, emails and many more information about their patients should be saved in a good manner.
All these things must be done by an experienced person like the CIO because as we know that the FMP telephone was installed in the 1980s and frequently needs an engineer to rebuild it (make it modern). Then CIO will introduce a new IT system by introduce their goals and targets to have an electronic booking system, have NHS care records service and have an electronic transmission.

I think fumble practice need to respond to ongoing changes in the environment, political, social, economic and technological conditions in which organisation operates.
The current system serves various functions. First, the paper-based filing system allows records and files of patients to be recorded before being stored in the basement of the institution. Second, the institution has various PC’s that the administrators’ use for word processing.
Additionally, the practice manager uses the computers to process the payroll for the employees using management software linked to BACS limited. Finally, the institution possesses an old-fashioned telephone system for communication. However, the telephones were installed during the 1980s thus require frequent maintenance.

Fambul practice is an institution that has been using the outdated system in their daily operations. The facility involves seven partners who have conflicting views on managing the facility. The managing and finance director, Dr. Bo-cooper and Dr. Coker respectively, are senior partners who still cling to the traditional information management system of the facility.
The two doctors face opposition from Dr. Samara and the late Dr. Dean, who want to introduce new technologies in the bid to improve the healthcare provision of the society. The system involves the traditional file storage system that inconveniences both the patients and staff of the hospitals. Patients’ complain of difficulties in booking appointments as well as cases of missing files. Employees have difficulties in maintaining the large records of patients and yearn for automation of the hospital systems to ease their work.
Discuss how you would relate theory to practice
In my opinion, the most useful things to me is that ‘ to understand and more details on existing theories, if I want to create theory to practice. I am not focused on something to understand or whether it is resonating and ask myself, can I put it to use? Sometimes, you know the result without finishing your work, because if we had focused on more the way we present information, then translate it into the practice, the result should be clear for you, that’s why is better to always put relate theory to practice.
The technologies adopted by the hospitals are required to function with minimal or no errors to effectively enhance the treatment of patients. Fambul needs a CTO to select new technologies to be implemented as well as monitor and evaluate their performance.
The CTO is supposed to analyse the functioning of the system critically, before selecting the system with the best ROI. In the process of determining the best technologies for the institution, the CTO ensures that the hospitals are financially stable as well as operate using the best equipment possible.
A large percent of the current healthcare industry has adapted technology in the process of improving their services; therefore, it is appropriate for medical facilities to employ CTOs who would help in ensuring that the IT resources have been well utilized and that the information systems are efficient in their operation.
The establishment of a new system in Fambul would help enhance the provision of services to patients; for example, automated systems would improve the speed in which clients are treated since it will ease the process locating their information and appropriately directing them to the departments that they are supposed to receive their medication. Additionally, the workflow of the entire facility would be organized thus making it easier for doctors to prioritize their treatment plans for patients depending on urgency.
Application: How does the theory and practice relate to the case study?
In believed that the provision of health care varies (in a number of ways, the amount of money spends, qualities and type of service) from one hospital to another, from one GP to another GP, from on City to another part of the country. Not all variation is bad, some of them linked to the patient and some of them is unwarranted (patient receives treatments they don’t need or denied access to those they do need). that is a negative impact and leading to costly errors and decrease in quality of the service. If these variations addressed, then there is potential to save lives, diagnose ill sooner and preventing emergency admissions.
The role of the CIO in Fambul medical fraternity involves the identification of the efficiency of the current healthcare information system, management of available IT resources, and education of the staff on the methods as well the significance of a well-structured Information system within the hospital.
Dr. Samara, the current CIO of the hospital has identified that the hospital needs to involve experts in updating the system. The problem that the CIO is likely to encounter is the possible rejection of his recommendations by the other partners. For example, Dr. Coker and Bo-Cooper, who are the most senior partners in the institution, are attached to the traditional way of the system and are therefore likely to resist the adoption of the CIO’s strategies.
Relate theory to practice in health services research is not happening without its challenges given by the typical training of researchers and staff to the in disciplinary nature of the practice. The most challenge is that, to make the staff and the researchers busy by engaging them in theory driven research, which is required organizational conditions that support staff and decision makers collaborating in the development, updating and testing of theory.
Moreover, encouraging staff to pursue a programmatic approach in research, because theory development and testing is strength in nature and offer promise for understanding of what happens in health and social care when interventions work to address as well as this approach helps staff to understand how funding agencies support health services research.

A successful Chief Knowledge Officer for higher education must possess hard technical skills and soft interpersonal skills. CKO’s will have two main duties; (a) designing institutional information portals based on a knowledge infrastructure and (b) championing an internal culture of knowledge sharing (Adams, 2001). [8]
The Chief Knowledge Officer (CKO) is responsible for an organization’s knowledge management, planning, strategies and implementation. A Chief Knowledge officer or CKO is the highest ranking KM practitioner in any organization in which the definition of its roles, skills and competencies are emergent (Weidner, 2005). The CKO is responsible for developing strategies and processes that facilitate knowledge transfer. Other CKO’s responsibility is to maximize the creation, discovery and dissemination of knowledge in the organization (Earl, 2001) and for the creation of processes and systems that harness information and mold it into usable business intelligence (Allbert, 2004). These rolls are essentials in 2015, those jobs are super challenging and business expectations are high, but CIO still struggles with the need to keep IT operations humming along with balancing and securely. In some business colleagues still IT is obstacle. CIOs jobs getting tougher. Meanwhile, The CIO who falls to keep hackers out of the mail flowing is in big trouble. CIOs plan to remain in the role of personal carers and foresee themselves as ‘digital strategist’ as well as focusing on managing contractors, IT service provider, risk, product development and cloud vendors.
On the other hand, managing the complete customer experience in both the external and internal facing of the building, innovating and operating technology is the biggest challenging of being CIO, CTO at the same time and make a balance, just by keeping calendar full and splitting between external and internal factor as well as by engaging all players involving in business to make sure the decision is being made the data available and dependent on your business need. Furthermore, the greatest benefit of being both is more effective and able to operate quickly and easy as possible to demonstrate new capabilities’ to customers (improve customer satisfaction, talk to the teams, advisors, partners regularly, retention continuously) and to scale and innovate faster than the competition as well as manage all aspects of the business. What I understand from the lecture is that everyone in the organization should have the training, monitoring of complaints to see the reality of each parson feelings, positions whether they are writing or wrong.
I think the most important things for the organisation to be successful, achieve specific goals, and objects are, they need tasks, people (managers and employees) and technology to gather information as well as to welcome complaints by encourage every member of the staff to ask for them ‘ complaints, this makes to improve quality and relations with customers because many consumers and business people are not like to expose themselves to the situation when they are asking them to make a decision, that is leads to make more pressure of arriving at a deal.

During last week we focused on traditional organisation, modern organisation problems advantages and disadvantages, the knowledge creations and the seven levels of information management as well as answering a question which was, ‘is LSBU a traditional or modern organisation?’ by researching about this university I found out that the university is a modern organisation in some ways like: eLearning, using technology to deliver products, education program and increase specialised courses. It is also traditional in some ways like: the structure, each school having its own dean, hierarchs the size and provide a higher academic standard.

I have learned from this lecture, research practice, continue professional developments, Moodle and social media. All of these are not complicated and are much easier to us in practice. In modern organisation we have to put CIO and CTO into how you achieve your business because each function we use has 1 or 2 directors and they relate to each other to form something powerful. [1]
I found out that the traditional university have better teaching, atmosphere, social life, free meal, societies and clubs. However, if someone want to study traditional university is better to go their if not for example if they want to study in a new building, study media, Global business, modern languages, IT related subject, etc. is better to find the modern university.

This week’s lecture relates especially to the case study because the GP kept on using paper and pen to make an appointment, cancel one or contact a patient. The longer you were patient at the FMP the bigger your envelope and storage space as so far the PCs has still not been networked. This is because IT and knowledge will not become more important; partner’s personal computers either lie dormant or at least are not used for practice business.
When a certain thing is created, it will be used for many things. The reason why they don’t have networks is because all their past families have done the same thing so they don’t have the knowledge to install the FMP and make it modern and so to do that they will need a lot of money to hire an experienced person to do the job.
The management of the hospitals can finish Dr. Dean’s work by interconnecting the computers within the hospitals to allow access to information within and outside the facility. This can be achieved through the instalment of both the intranet and the extranet.
Advising the partners to share their knowledge and opinions about the management of the facility
This situation involves making sure that the senior partners discuss ideas and facts before reaching an agreeable decision. This helps to nurture unity in the organization, thus enhancing better cooperation of individuals in managing Fambul facility.
Implementation of Dr. Samara’s new telephone systems that directly connects to computers
This act involves linking the new phone systems to computers to enable the staff to record additional information about patients in their databases. This ensures that patients’ records are appropriately updated.

I believed that the trust is essential to develop solution because it is helping to understand, relationship and communication. It needs to have very special skills to manage the situation because if we need to be successful and sustaining personal relationships, constructive relationship in business, to win and retain a customer, we must avoid conflict and we must able to step back and come back with a new idea from our emotions.

Over the last week we focused on Human aspects of information Management, the role of IT trust and Intellectual Capital, which is made up of three elements: human, social and organisation capital, Trust, the importance of trust, which is about accepting vulnerability and is belief in the honesty and justice.
There are three kinds of trust typology: companion, competent and commitment trust. To identify suitable empathic information-gathering questions. For example, Rousseau and her colleagues offer the following definition: “Trust is a psychological state comprising the intention to accept vulnerability based upon positive expectations of the intentions or behaviour of another.”[1] Similarly, Lewicki and his colleagues describe trust as “an individual’s belief in, and willingness to act on the basis of, the words, actions, and decisions of another.”[2]

We have ability to capture information and build knowledge together, make stronger and develop each other by working in teams. As nonka stats that we can do everything from sharing knowledge/information together and to fix of any problems they might be facing in the organisation.
Social capital in organisation is knowledge created, can be developed and transfers by relationships (companion), partners and suppliers because each individual ‘s personal knowledge, experience is transformed into the organizational as a whole.

Companion trust
Several managing partners are friends who have trust beyond the place of work. For example, Mrs. Brown’s child is the goddaughter of Dr. Bo-Cooper.
Competent trust
The partners show their trust and belief of Dr. Samara when they appoint him as the CIO to help in the modernization of the institution’s new system.
Commitment trust
The stakeholders of Fambul have little commitment trust with each other since they do not share information that is supposed to help them form a strong management team. For example, Dr. Coker and Mrs. Brown always have some anxiety and tension between them due to their varied opinions of the way things should be handled in the institution.
Social capital
An evaluation of the case study reveals that creating a friendly environment for both the employees and the patients will help improve the healthcare provided to the patients. Dr. Samara, who has been appointed as the project manager and the Chief Information Officer of the hospital, has identified that patients frequently complain about the quality of services provided by the institution. His documentation suggests that the modernization of the system will help to improve the provision of healthcare.
Organization capital
This element helps in the identification of the information system that the facility has installed in the process of managing the institution. For example, the hospital possesses an extremely outdated information system where the files and information of staff and employees of the institution are still file based and are stored in the basement of the medical facility. Additionally, the management incorporates an outdated communication line as well as payroll system in their management process. Dr. Samara has identified the major issues that affect the current management system of the facility and has offered various suggestions for the improvement of the system. He has recommended the introduction of experts who would help update the system.

I think Information Management is essential to protect, store, organise data as well as destroy unwanted data through the information life cycle. The process looks like simple, but there are common mistakes that prevent information management from being successful.
-As we can see that information management is important for every business. However, when you believe that information management is the only way to being used in big business, is wrong in a legal sense and in a business, the data protection principles are relevant in government regulation to business of every size as well as those businesses big or small, that actively manage their information and organising are far more successful.
– In addition, waiting too long before updating a system could lead to loss of your information as well as for the old system difficult to cope with the new system because the growth the information needs to be anticipated and factored in as part of long term planning. That’s why, it important to installing a new system and updating as soon as possible to not loss your data.
-Moreover, Backup or data recovery system is vital for storing information that is not currently used, especially if your business handled a lot of data, as well as archiving is a similar process but less organised than information which has been backed up individually. When the original data is stolen, lost or destroyed, the business can retrieve the necessary backup file rather than through the archive materials, this could avoid costly delays.
-Many businesses are not realising and not aware of that social media is important like any form of communication, advertising, data, information, updating needs to be organised and managed properly and required a formal policy as well as a legal liability for social content.
– Furthermore, many organisations believe that the information managing is the only one for the IT department, which is not true because business leader must consider carefully about information strategy and implanted across the entire workforce.
-Many organisations and enterprises are still using paper documents as part of their business process, printing it out and keeping several copies of particular document .it could lead to loss, errors, and data theft. That’s why, they need to implement discovery plans to converting a paper documents and records into a digital information management by using centralized or electronic storage, which provides accessibility of the company and respond quickly without disrupting or can be more easily to find any data/document in case of any event or unexpected might be happening.

I learnt about trust within an organization is necessarily agreeing, understanding and listening properly not judging another person, which is makes you to understand how the other persons feel and discover what they want, then you can make a decision to work with that person/ business to trust them and do exchange idea.
This element is extremely crucial in the medical practice since it indicates the work the specialized hospital staff does in the processes of improving the professional care provided to patients. In this study, Dr. Samara and Dr. Dean are the two individuals who seem to possess this trait.
The employment of Dr. Samara allows him to bring about his idea that involves the introduction of a telephone system that will be directly linked to the computer systems of the hospitals. His concept aims to be advantageous in two main ways.
First, it will enable patients to book surgery appointments from home. Second, the Caller Line Identity installed in the systems will enable the staff to record patients’ details during call sessions including name and address, thus enabling the staff to confirm as well as update the patient’s records.

During the last week, I have learned the requirements of a new system that must within the FMP. In my opinion, the gathering information and the analyst are the first steps in developing a new system, gains a clear understanding the interfaces to the legacy systems and how the existing system works because it is not always easy to replace existing systems and simply wrap them in new code. So they need the process of changing the program code is important to understanding the internal work rather than the way how people do their work.
As we know that the FMP old system is the traditional system based on paper documents, forms and files. Whichever it is, the part of the system meets the needs of the people who use it familiar and comfortable with it but, there is some part of the system that no longer meets the needs of the business for example physical attend for making an appointment. Data can be used to benefit patient care to access information by manipulating and handled intelligently because data is the most essential tool used by healthcare provision.
Discuss how you would relate theory to practice:
The FMP medical practice should through review of their organization requirement and processes to identify what is working well, what is not working well and which aspects of their business need improvement. Then that requirement will then be mapped into the shape/ project plan, which will drive the remainder of the implantation.
The Fumble practice should begin to design, develop, their implementation team by using the plan to build the system, which is used in system functionality and business process in the project plan.
Then, teaching the practice staff the best practice to deliver the functionality of the system by using the data migrated from previews system and tested. Then results should be reviewed and accepted by the CIO and the manager.
Next, Knowledge worker (working from the knowledge obtained in preview steps (experiences and resources) (with internal and external operating environments) is ready to make and provide the exact final transition to the system that is required to meet the organization objects.

Every organization has competitors, which knows your plans as you are in knowing their plans because great organisation comes up with a great products and wise managers.

Competitive intelligence (Time, cost &badges, quality) is the core of competitive strategy. Competitive intelligence is firmly to decide on something to get solutions to those principle factors that are an ongoing competitive advantage. I believed that, If we want to have a knowledge about a conflict, identifying ethical ambiguities and address any ethical any issues we must learn about the pressures (to be ethical or unethical),which is exist in the current CI environment which is under CI professionals work.

It is important to understand the difference between intelligence and information.
It is a bit of data of the company, people, number, statistics and product strategies, but intelligence is the information that has been the analysis and used for decision making. Making business decision: To cope with the business environment, we need to learn from the past, our mistakes because the present is the challenges and the future is an innovation, this is why most of the organisation love changes and develop flexibility to make their future bright.
Identify gaps: In system analysis and design the functional and non-functional requirement are conventional categories, while usability is ignored by the practise and many organizational in a system development project. [4]
Internal information: As we can see from fumble practice that is driven by internal factors to grow, change and the process of replacement from old system to take advantage of new technological developments. The process of gathering information could offer enhance their usefulness to management and workforce. [2]

During the last week, I have learned about decision making in an organization. As we can see in Fambul practice, they need to learn about knowledge (to get the tacit knowledge) for their systems NHS and business to provide a better service by using a better method analyse information in many different ways to prove a good health care, which they need a good structure with the right decision.
Discuss how you would relate theory to practice
Another component is communicating with the staff and the patients in a different way, for example, via mail, post, letter to sort out relevant data in a patient’s history by using electronic database (diagnostic support software system can easily provide help to physician to a list of possible diagnosis and identify possible diseases) this could help to write a prescription and to monitor the patient’s progress.
I believe that the Transaction Support System in DSS is very important to all organisations and will bring the wealth of knowledge to physicians to meet challenging patients because it concerning about how the information technology can help the health care with information storage, organized, processing and updated it.
In my opinion, the strategically level is important too, for example, if the practice want to reduce the harm for the patients, reduce errors and to increase the safety of the clinical environment, it must have a good structure, implement the actions, the skills of operators because what I understand from the lecture is that the healthcare is a delivery system, which is well established that need for innovative methods to translate research result into practice. Reason for choosing the CIO:
A large percent of the current healthcare industry has adapted technology in the process of improving their services; therefore, it is appropriate for medical facilities to employ CTOs who would help in ensuring that the IT resources have been well utilized and that the information systems are efficient in their operation.
The establishment of a new system in Fambul would help enhance the provision of services to patients; for example, automated systems would improve the speed in which clients are treated since it will ease the process locating their information and appropriately directing them to the departments that they are supposed to receive their medication.
Additionally, the workflow of the entire facility would be organized thus making it easier for doctors to prioritize their treatment plans for patients depending on urgency.
Before deciding any decision, every organisation should ask a question (how can we design and use DSS in the way to minimise the risk and maximize the benefits of the patient?). When the practice is modernised, we could see that the decision support system plays an important role in making decisions, assist physicians in making clinical decisions, to improve the quality of medical care as well as can decrease health care costs by providing a faster diagnosis.
However, using inappropriate DSS with no concerns about the ethics or malfunctioning could make jeopardize of the patient and fail to achieve the level of diagnostic performance as human experts because new technology with new application will need more concern about ethical for expanding and modification.
Moreover, by the growing availability of technology could help the Fambul to apply medical knowledge and provide decision support for a particular diagnostic for example, interpreting pulmonary function tests, managing the use of anti-infective agents and analysing electrocardiograms. [3][1]

The technologies adopted by the hospitals are required to function with minimal or no errors to effectively enhance the treatment of patients. Fambul needs a CTO to select new technologies to be implemented as well as monitor and evaluate their performance.
The CTO is supposed to analyse the functioning of the system critically, before selecting the system with the best ROI. In the process of determining the best technologies for the institution, the CTO ensures that the hospitals are financially stable as well as operate using the best equipment possible.
Every organization has their own component, so what I understood is the Communication: such as meting system, organising, instant messages which need web client server are the most component.

FAMBUL MEDICAL PRACTICE
There is many problems are facing to fambul medical practice. Once I used Swot analysis I understand that the practice manager can do reorganise, redistribute and interrelate by understanding the internal and external factors, which has negative and positive effect.
FMP has been in experience for 82 years and can be availed of staff experience and people in the area trusted them by herding their motto, which was ‘Our Heart is the People’. On the other hand, code of conduct has been followed in practice. In a dynamic state, most essential transformation is cost concerns, legislative reform and quality issues.

To performance transparency issues and to supporting executives at healthcare organizations on technology, the technology and information consultant should have taken an objective top management approach into account because by design/ increasing the IT technology and developing new system, allowing managers to identify the business function, marketing, promote transparency throughout the system, establish management culture, target areas for improvement as well as secure funding for its implementation.[1]

Intelligence
The case study reveals that the employees, patients, the medical practitioners and the stakeholders are unable to provide better healthcare due to the lack of an up to date information system.
Design
In trying to solve the problem, there are different alternatives to the problem. The partner’s suggestions including selling the practice as a going on concern, purchasing an existing and tried system, continuing the GP surgeries stalled program or investing in a bespoke information system. Dr. Samara, the newly appointed CIO recommends the use of an expert to update the system.
Choice
According to my opinion, the use of IT systems requires adapting systems that have already been tested in other institutions and proved beneficial. The best choice for the facility would be to select the installation of a new system by IT experts.
Implementation
Dr. Samara will be responsible for providing information on how to link the IT strategy to the current system to improve efficiency.
The FMP practice should have revising objectives and reviewing in terms of changing demands to maintain effective communications with the staff, working groups and larger working group by selecting, motivating, training, managing budgets and evaluating accomplishments.
The clarifying mission, policies, planning, the development and objective are the skills that required to manage people. The Manager has a status that leads to interpersonal relations and from this comes access to information. It enables managers to devise strategies, making decisions, and implementation (Mintzberg, 1988).

There are four phases of Simon’s model of decision making. Design phase, we have to identify the points, search for alternatives available at each points, a range of alternative outcomes at each point as well as estimate the result and the costs of action and finally, analyse value to choose what the next course of action will be.
Intelligence phase: when a system uses electronic data, which allows collecting clinical data in electronic form by searching, monitoring, scanning and transferring the data from one
system to another. Also, it can be able to identify the problem that can be importing them without the use of paper this resource available to achieve the quality of the service and important objective which is required to achieve in an organization.
Designing is a process to develop objects and goals for the organization by defining and clarifying organization structures, function and quality service improvement. identify changes needed to move from the current situation to the next. One of the difficult steps in the design process is to develop the alternatives. For example, the time and the location is the most crucial for an organization, when the planning to do something.
The choice phase is important as well in Simon’s model, when the organisation aims are determined, some activities required to transform the plan and aims into action by selecting alternatives of action to evaluate the outcome that may occur in the future.

Medical technology is rapid growth in modern society, which is improving the quality of health care and economic growth (consumer demand for better health). That’s why many organizations offer the training for their staff to learn how to use the technology to provide a fast, complete cure, increase safety, diagnosis and; minimise side effects of the patients.
Decision making is something/process that happen every day and it is something that you cannot avoid in life for those, who work in an organization so, before making any decision, we have to identify what the problem is, use the strategic questioning where we can analyse the feeling to the situation that we would like to take.
Discuss how you would relate theory to practice
Nowadays, the health business is become dangerous when it is not managed properly because small mistakes that leads to big problems to some once life in a way impossible to change it back. That’s why that the health care provider can be the ones that the patients trust, obey by ethical, legal standers and rigid professional because the reason why the patient goes to the doctor is to get better but, sadly not always happen.
As we can see that most of the organizations are arranged around groups and teams is
management’s assumption because sometimes in positive side, the group decisions are better than individual decisions because many members contributing their diverse experience, knowledge and skills into better decisions .However, the conflict has happened during the disagreement of the member, which leads to make the wrong decision.
We have to classify the problem by dividing the problem in a small area as Simons model mentioned that, once we find the objective and what define our goals, then we will find the problem easy. The main problem of the practice is: no clear objective, staff (to improve customer service), a paper based booking system, Information system not been used, cost issues, and historical data.
For the practice, we need the structural design to develop the system because many people are involved (management, staff, Dr, nurse, patients and local people). The structure design, which is done by the operating system to do a booking system, data entry, product shipping and maintaining patient records. The tactical design is done by those who responsible for purchase order, production plan and production order.

In order for the Fambul practice to make a successful decision, their decision need to be include programmed and non-programed. Sometimes programed (virtually automatic) decision is much easier because nonprogrammer is a (generally unexpected.) decision making. On the other hand, non-programmed is easier by taking a number of steps to improve planning. In order for the Fambul practice to do this is, to educate their employee, not for improving their position and promote hiring, but it’s also helping the practice time and some money on training through outside courses. as we can see at practice different people at different levels by having their different opinions make a problem to make a best decision and to progress to a modern practice.
If we want to keep the practice working continually, we need to change/combine the problem into the social, ethical, logical and political issues as we can see in the case study when Trisha went on holiday, someone needs to contact her because no once have an experience like her that’s the social, logical and communication problem with this practice.

In the representation of the problem situation, I would first draw a large circle at the centre of the page. The circle would contain a building sketch with a plus sign to represent the medical practice, a clipboard icon to represent the file storage system, a telephone system icon to represent the old telephone technology, an old picture of a computer to represent the management software that is linked to BACS limited. The outside would have to be evenly distributed with different sketch people representing the stakeholders, the patients, and the employees. The government would also be part of the external circle and would be represented by an icon of a house with a flag post attached at the front of the roof. Arrows would have to extend from the people outside the circle pointing towards the centre of the circle. I would have to create an organizational chart besides the shareholders to represent them according to their rank and include their detailed opinions and ideas relating to the current system. Dr. Samara would be situated beside the organizational chart as an individual who had to fill in for Dr. Dean. He would also have an arrow pointing to an expert he recommends to the medical practice. Additionally, his recommendations would be included in a text box beside him. The government icon would have a text box beside it displaying their policies regarding implementation of IT structures in hospitals. The patients and the employees would have a text box displaying their complaints about the system. Individuals’ role in the problem would be indicated below them in text boxes whose arrow would be directed towards the middle of the centre.

During the last week, we learnt how to doing a rich picture by understanding A Root Definition, which is a first step of Soft Systems Methodology (SSM) of the System that we are going to designing, analysing and studying. It is a structured description (statement of activities) of a system in the organisation.
CATWOE analysis helps the system to identify and categorize all stakeholders such as people, environment, processes in the proper formulation of a Root Definition. [3]
Discuss how you would relate theory to practice
The first thing, we have to put the name of the system of the FMP if not we cannot understand the purpose or function and we become confused at who is going to perform the activities and what those activities should be. That’s why, we come up with the Root Definition of the system of which supports each transformation. [2] For example, if we have X, Y, and Z. Then, we can define each of these as follows:
X-The ‘what’ is the system do, which means that the immediate aim of the system,
Y-The ‘how’ means that how the system does it, which means of achieving that aim,
Z-The ‘why’ means that why the system is being for longer term aim of the purposeful activity.[3][1]
CATWOE MODEL
C = is the stand of the Clients, System Beneficiaries, victims or Customer.
A = is the stand of the Agents or Actors (who carry out the activities envisaged in the system).
T = is the stand of the Transformation process. It transforms the input into output.
W = is the stand of the Weltanschauung or World view that lies behind the root definition. It is putting the system into its wider context as well as highlight the consequences, then, input the system in a place to make the world environmentally safer.
O = is the stand of the Owners or person who has sufficient formal power over the system.
E = Environment constraints, which is include things such as regulation, resource, limitations, ethical limits. [1][3]

The main problem of the FMP rich picture is to provide efficient service and the problem of the practice, which is the managing director(Tricia) because there is a real conflict among the staff, the conflict in the exchange of information over the phone to get a better service of managing the practice.

This week I have learned two more things, which is important in management such as:
Social Structure: once the practice creates the work group department. it will be helpful to encourage patients to put their views and ideas in practice, local healthcare services and the NHS. For example, put their view about their health care needs as well as future development, provide feedbacks about the services provided, encourage them to work in partnership and health issues. [1]
Physical Structure in Practice management
The practice manager: look after the day to day management and discuss any non-medical problems, receives suggestion by the practice and deal with enquiries.
Receptionists: responsible for dealing with telephone calls, enquiries from everywhere and give advice, hospital appointment and doctors to run the surgery smoothly (Checkland, 1999, p. 255).
Prescribing Clerks: I think the practice staff should be trained and responsible for the computerised accounts, repeat prescription.

I found out some key aspects of the organization structure chart such as:
1. Division of work: I think, each person in an organization should be able to determine what is their responsibility by providing the chart, then, the practice function become more efficient and all the tasks will be completed on time because no wasting time and resources when too many people work together and share responsibility.
2. Line of authority. As we can see at Fambul practice is hierarchical structure and the line of authority is a formal structure of relationships in which the authority is legitimized for performing the tasks, because is characterized by rigid and reflected in an organization chart from top to bottom. For example, Tricia is the practice Manager of a small department in a non-profit organization. She is anxiety, lack of communication with a practice partner (disagreement with Dr BO Cooper, who is a finance Director and deals with the tight budgets as well as financial planning). And also she is struggling with a tough decision. She is got own software and responsible for payroll.
3) Span of control& Delegation and decentralization: is refers to a number of subordinates who is directed by one supervisor. when the number of subordinate is increased then the shape will change from pyramid to flatter and the delegation is responsible to transfer authority for directing the task to one or more subordinates it will leads to decentralized or removed from one occupied position as well as it has the effect of transferring authority to lower level and the span controlling will be widening at high level.
4. Departmentalization: The FMP organization has a very strict guideline for termination and it is necessary in Fumbul practice to provide the specialization and division, which is referred to all departments as well as it requires to facilitate the specialization of activities [2].

The Fambul practice needs to provide the organisation chart, which is a point of references and improves the direction of communications. It will help patients to see how they fit in the big picture as well as increase efficiency and maintain the balance of the medical practice. The structure (the anatomy of a practice) refers to the formal organization, schedules, plan and procedure (the practice’s physiology) that (people)hold it together to carry out the task [2].
In FMP medical practice case study, the process of the traditional organization was usually undertaken by the team (Dr, Nurses, staffs, receptionist, and managers), however, if one person of the team members decide to leave the organisation, it could affect the efficiency of the process for example when (Tricia went on holiday) because of the loss of tacit knowledge. So I think by using SSM could provide the information about tacit knowledge, and help us to approach the overcome of the difficulty in working with this circumstance.
