Introduction to nursing

For nurses, our company per say is the hospital and organisation that we are working at. Organizations such as singhealth, ministry of health, National healthcare group (NHG) or others and the hospital that we are currently attached to.
Singhealth is a largest healthcare organisation and institution in Singapore. The organisation was formed in 2000 and consists of 4 public hospitals which includes KK women and children hospital (KKH) as well as Singapore general hospital (SGH), 5 national speciality centres and a network of 9 polyclinic in the country. (Singapore health services, n.d.). Singhealth vision is: ‘Patients. At the heart of all we do’, which helps uniting their minds focusing on delivering the best possible care for their patients (Singapore health services, n.d.). Hospital profile
Singapore General Hospital (SGH) is the first and largest hospital in singapore and it provides affordable specialist care for patients, training for doctors and other healthcare professionals and it also conducts research to bring about better care for its patients (Singapore General Hospital, n.d.). There are a few specialist centres co-located on the SGH campus, facilitating a multi-disciplinary approach to treatment for patients, it practices team-based care as provided by clinicians, nurses and other health professionals (Singapore General Hospital, n.d.). SGH caters up to approximately 1 million patients each year, accounting to a quarter of the total acute hospital beds in public sector and one-fifth of acute beds nationwide, with the help of 10,000 strong workforce (Singapore General Hospital, n.d.).
Introduction to internal medicine
The department of Internal medicine caters to patients with wide spectrums of clinical conditions and provides cross-speciality care for them (Internal medicine overview, n.d.).
Roles and responsibilities
To be a staff nurse, is to be able to deliver proper nursing cares and practices to our patients at any time without fail. Staff nurses are to be equipped with proper nursing trainings and sufficient nursing knowledge to better serve the patients. Able to manage and plan care management of individual patients using the 4-prong systematic approach to nursing assessment which comprises of assessment, planning, implementation and evaluation. Staff nurses will be required to follow doctor’s and medical officers’ rounds to further understand the care practices and orders that are made for the patients and als, carry out the treatments, procedures and/or investigations. To be able to work 3 different shifts; morning, afternoon and night shift, as per scheduled by the nursing clinicians. As nursing is considered a service-based job, one is to ensure patients’ safety as well as comfort throughout the job. Also, ensure to demonstrate professionalism and competence in carrying out tasks and procedures. Ensure that working environment stays neat, tidy and clean, as to further improve patients’ safety and condition. One of the task that a staff nurse will need to do is to give patients their medications, it is important that staff nurses is to have strong foundation on their pharmaceutical knowledge before attempting to deliver medications.
One will be based in an internal medicine ward located in Singapore general hospital to provide timely and appropriate medical services for all patients in the ward.
Job requirements
To be a staff nurse, it is required to have a diploma qualification in nursing from any polytechnic or known legal schools as recognised by the Singapore Nursing Board (SNB). At least 2 years experience of clinical practice in acute hospital settings and other licensed medical facility is mandatory. One have to be registered with SNB to obtain a certification that states that he/she is certified to work under singapore’s public and private hospitals. It is advisable to have excellent interpersonal skills, good customer service skills and is able to use english and various other languages to communicate efficiently. It is also required for a staff nurse to have a basic cardiac life support (BCLS) certification. Staff is required to be able to operate machines of relevance and be competent in able to measure another’s parameters such as, blood pressure, heart rate, oxygen saturation, temperature and breathing. Staff nurses are often needed to stand for their entire shift which can last around 8 to 12 hours (depending on the shift), they are require to be physically and mentally fit for shift works. As staff nurses are unable to choose the department or ward they are arranged to, all applying personnel will need to prepare themself for any kind of work environment, as not all wards is air-conditioned and that they might be arranged to a infectious disease ward which requires them to gown up in personal protective equipment (PPE) for the entire shift.
Psychometric test(s)
Mini-mental state examination (MMSE), also known as a Folstein test, is a 30 point questionnaire that is used extensively in research and clinical settings to measure cognitive impairment (Mini-mental state examination, 2018). It can also be used to assess dementia in elderly and to estimate severity and progression of cognitive impairment and the cognition progression over time (Mini-mental state examination, 2018). It is common that people working in the clinical area giving this test to people to test their cognition.
Generalised mental condition test is a test that is made associated by Ivan Goldberg’s mental disorders tests and tests by Mental health America. It is made to access the mental status of people who wishes to apply for this job scope. As a staff nurse, it is required for one to interact with immediate peers and other health professionals from same or different departments frequently. A staff nurse is required to collaborate with nurses, doctors and other medical professional such as, physiotherapist, pharmacist, etc. Staff nurses are to pass report to other fellow nurses in the same department of different shift to further care for the patients and update patients’ families, relatives and friends of their conditions. They are required to follow up with other healthcare professional such as physicians and nurses of the other departments on patients’ conditions, treatments and procedures. Inform the nurse clinician in-charge on any changes made or happened during the shift. The job also requires communication with patients to further understand their current statuses, conditions and feelings.
Rules, regulations and authorities
First and foremost, patient confidentiality is by far one of the most important and common said rule. To be able to allow patients to see solace and comfort in a healthcare professional require patients to trust the healthcare professional. As a staff nurse, we are to protect information that is given and made known to us due to the profession, not allow it to leak out to the public or anybody that is not suppose to know and to keep it from those specially mentioned by the patient. Afterall, nursing is a customer-based service centred job. We are to seek permission from the patient and/or their family members before enforcing every procedure and practice upon the patient. We are to follow all the safety procedures and practices and remember them by heart, to ensure the safety of our patients and not let their health be further compromised.
In nursing, there is a hierarchy which we are to follow. The highest rank belongs to the nurse clinicians which are also known as the sisters of the ward. Staff nurses are to report to them when asked and/or if there are major changes required or made. Nurse clinicians are the people who will communicate with the hospital and other higher ranking healthcare professionals for various issues. After nurse clinicians is followed closely by assistant nurse clinicians. They are the ones which is in charge when there is no other nurse clinicians is on shift, and help out nurse clinicians with their duties when nurse clinicians are on shift. Nurse clinicians and assistant nurse clinicians are the ones that are accountable for the other ward, they make the decisions and ensured that the actions are carried out by all nursing staff in the ward. Subsequently, there are the nurse practitioners. Nurse practitioners’ duties are that of a medical officers; as nurses, we are not allowed to prescribe drugs but as a nurse practitioner, we are allowed to. Then there is the Senior staff nurses and staff nurses, which roles are the same.
